Post Acute Partners, a New York City-based health-care services company with operations in four states, has completed its acquisition of Elderwood Senior Care.

The senior care company, which was owned by the Chur family, includes nine skilled-nursing facilities, six assisted-living residences, two independent living communities and an institutional pharmacy. The purchase price was not disclosed. The deal was previously announced 2011.

Elderwood was founded in 1978 by Carol and Robert M. Chur, with one facility in Williamsville. Today it has 19 facilities in Western and Central New York, employing more than 2,900 people and caring for 5,000 patients and residents annually.

Post Acute Partners, which was founded in 2010, plans to keep the Elderwood name on the facilities.
The Chur family will retain two assisted living residences: Elderwood Assisted Living at Penfield, and Elderwood Assisted Living at Heathwood. Those two locations will now be known as Heathwood Assisted Living.
